Enhancing Health Equity in Professional Case Management Practice for LGBTQ+ Individuals and Communities

Not only during Pride Month, but throughout the year, it is important to recognize that LGBTQ+ individuals and communities have health and human services needs that professional case managers need to understand and acknowledge to optimize the effectiveness of services while enhancing health equity. To achieve these goals, case managers need to ensure they understand key terms and concepts in the LGBTQ+ community, such as gender identity, sexual orientation, gender expression, and other terminology. Professional case managers must also incorporate into practice the applicable ethical principles and guidelines to promote inclusivity and equity while working with LGBTQ+ individuals and communities. In working with LGBTQ+ individuals, there is a need to acknowledge and understand the significant health disparities that disproportionately impact this community. This presentation will incorporate effective strategies and techniques that improve communications with LGBTQ+ individuals. It will also describe optimal and proven care coordination strategies and techniques to meet the comprehensive physical health, behavioral health, and social determinants of health for LGBTQ+ individuals and communities.
